About this provider

Lean On Me Community Care Services Ltd aims to provide a quality care service to people in their own homes. This will be carried out by the delivery of personal care and associated domestic care that is constantly monitored to achieve a standard of excellence that includes the principles of good care practice. Our goal is to be the Home Care Service of choice in the local community.

We provide domiciliary care to people in the community on a daily, weekly, overnight or ‘live in’ basis to older people with physical disabilities, people with mental health needs, homecare support services to people with Palliative care needs.

Personal care:
Ÿ Assistance with dressing/undressing - getting up in the morning and preparing for bed at night
Ÿ Assistance with bathing, showering and washing
Ÿ Assistance with skin care, washing hair and cleaning teeth
Ÿ Assistance with bed bathing
Ÿ Assistance with toileting and use of incontinence aids
Ÿ Assistance with medication and health related duties (in accordance with the written Care Plan)
Ÿ Night sleepover or sitting service and day sitting services Ÿ Assisting with the preparation of food, shopping, cooking, washing up and menu planning

Social care including:
Ÿ Companionship
Ÿ Letter writing, reading and managing correspondence
Ÿ Escorting to medical and social appointments

Domestic care including:
Ÿ Cleaning and general routine household tasks
Ÿ Shopping
Ÿ Laundry (including incontinence laundry)

All tasks carried out by Care Workers will be agreed and set out in writing in the Care Plan/Contract.
