London Care & Support Team

About this provider

London Care & Support Team was established to provide services for two main service areas.

A Short Term Service: (3 Month) step down process from hospital to independent living. This process will involve the person being accommodated where daily living and some reintegration social skills will be enhanced and developed.

The process will have input from an Occupational Therapist, Social Worker, Fitness Instructor and the possibility of Legal Advice.

Long Term Service: people who have completed the above short term will be assessed and then either
A.  Live independently or
B.  Be placed in Long Term Supported Accommodation.
