Hooklands Care Home

About this provider

Residential Care
The Home’s main provision is directed towards a long stay residential care service. We provide 24 hour care, with friendly trained staff, who will support and encourage residents to continue to enjoy the lifestyle of their choice. Individual needs are met by holistic care planning through direct consultation and the rights of residents are balanced with duty of care. It is expected that the great majority of residents will live at the Home for as long as they wish to, and for as long as the Home can continue to meet their needs.

Dementia Care
The Home has chosen to offer a service to older people who have care needs as a result of mild to advanced dementia. The Home is able to support residents with dementia. All of our staff will undertake dementia training to enable them to provide positive and appropriate care.
The Home expects to enable residents with dementia to have a settled and supportive environment which will respect and allow people choice, dignity and as much independence as possible. Effective links will be retained with the local Mental Health Team and other agencies so that best practice and expertise is accessible for both staff and residents.

Respite Care

The Home will provide a short stay service – respite care – for a small number of people with similar care needs to the long stay residents. For people using the respite care service will want, as far as possible, to maintain the routines and levels of independence to which they are accustomed when living in their own home. Through the agreed care plan, staff will do their best to ensure that this happens and that the transitions between the home and their own home are smooth and problem-free.

We have a weekly visit from a Hairdresser. Resident outings are a regular part of our activities schedule.

Communion is held at the home regularly. We have external entertainers come and visit regularly and we also have many In-House activities arranged to meet individual needs

Our Health Care Assistants will treat each individual with privacy, dignity and respect and promote independence and individuality.

Care provided include personal care, health and well being, social and emotional support and hydration and nutrition.

We will ensure a safe environment with adaptations to meet individual needs

All staff are encouraged and supported to complete the Diploma in Health and Social Care and attend statutory and mandatory training.

Staffing levels will be assessed against individual dependency levels
