About this provider

We provide a care centre for individuals whom have long term conditions. The service has been established for 30 years and has been central to the local people of Scunthorpe.

Activities are offered to outside members of the public. We directly deal with the NHS trust and offer step down facility for patients whom are awaiting social input.

If placements are needed we offer a 2 hour assesment . Please call 01724 289000 or directly the bed Manager on 07984734445. Social workers and family members are supported throught the process of placement.

What makes our service unique, is that we offer six weeks support for a new client and families. We also assist the clients in rehaiblitation , if they wish to return home.

Our staff are trained to the highest standard and also in long term medical conditions such as COPD, Diabetes , management of medication, palliative care.

Day care also offers families a option to have a break from care. This service is beneficial to individuals whom want to remain at home but require extra social input.
