About this provider

Active Prospects supports people with learning disabilities, autism, mental health needs, acquired brain injury and those with complex needs and behaviours of concern, to live full aspirational lives. Our team of 250 trained and experiences staff support 300 people to have choice, freedom and control over theif lives.

We provide just the right amount of care, so that people can live as independently as they are able, with support on hand when they need it. Active Lives is the over-arching strategy that brings together all our different approach to underpin our support offer, to enable people to lead aspiring lives. We use uses a wide range of tools and theories to support everyone who meets our core aim, regardless of any behaviours of concern, illness, communication difficulties, health condition or mental or physical disability; and these give the people who use our services added value and opportunities to lead an active life.

Supported Living services located in Ashtead, Banstead, Caterham, Epsom, Horley, Leatherhead, Old Coulsdon, Kenley, Redhill, Reigate and West Ewell, offer support from just a few hours each week to 24/7 intensive support for people with complex needs. We offer focused approaches for people with learning disability. We offer a mic of independent flats (often with communal space for community and therapeutic activities) as well as shared houses, where people choose to live as part of a household. We encourage and support a life that is full of opportunity, and involves people as valued citizens in the community of their choice. We do this by ensuring all of the following:

- We use out valuable experience to ensure choice and control underpins everything we do to support the people who use our services

- We listen to the poeple we support and help them identify their needs and wishes

- We incorporate all of this in their own individually tailored support and action plan, and review this with them regularly

- And of course we also work closley with community agencies, educational establishments and individual employers.

Our trained and qualified staff support people to live as independently as possible, this can include the following:

- Support with personal care, home and daily living skills

- Staying safe and staying healthy

- Community, family and friends, and social activities

- Religious and cultural needs

- Planning for the future.

Community Prospects services are regulated by the Care Quality Commission (CQC) and all of our services are rated 'good'.

There are certain things that we need to know in order to confirm that someone is eligible to apply for a service. We will come and meet with applicant, and complete an assessment of need with them and their family or carer; and will obtain confirmation of the support hours you have funded from your care manager/referrer. Prior to someone moving in develop a transition plan, support plans and a risk management plans, to ensure that our service meets their needs and it is clear exactly what we are offering.
